This can be built on something simple like Google Forms and embedded on a page on your website. Or you can go really old school and create a simple word document or Google Doc to send to new clients when you sign them. Make sure you collect all the information you need to start working.

This could be things like:

• Their name and contact information
• Link to set up a weekly meeting (if it's on ongoing client)
• Login and PW Information
• A branding questionnaire if you are doing design work for them
• And so on.....

Once the form is ready to go, save the link so you can easily send it to them when they sign your contract!
